We have gathered important links to documents that we suggest are valuable resources to assist with your emergency planning and preparation. They will also help you with starting Meet Your Neighbors (MYN) in your neighborhood.

What is MYN?  An overview of MYN – why have it and why it’s important.

  • FAQs on getting MYN started in your neighborhood

How to Start a MYN Neighborhood – this is an outline intended to help you set up and host the initial MYN neighborhood meeting.  Use what works for you and your neighborhood to get started.

Neighbor Contacts Form  – Each household completes one of these forms. It contains personal info such as name, phone, special skills, special needs, pet info and emergency contacts.  It is shared within the neighborhood only.  MYN does not collect personal info.

Neighbor Contacts Summary – (attach as backup to your map).  This is a summary of house numbers and each resident’s name, phone, pets, and special needs.

Buddy Form – share your information with a few selected friends or neighbors so they know how to access you and your home in an emergency.

Leader Form – Neighborhood Leader Formneighborhood leaders are to complete and return the Leader Form to MYN <MYN@OakmontVillage.com> or put in the MYN folder in the OVA office. It tells us the house numbers of each house included in your zone.

Sample letter – “Invitation Letter to your Neighbors” with date, time, location, and overview of what MYN is and what the meeting will be about.  All you have to do is fill in your meeting details!  

Evacuation Checklists

We’ve gathered some of our favorite agency’s checklists all in one place, including Pet Evacuation Checklists.  See which might work best for you.

Map Your Neighborhood Video – This is a YouTube video.  Although it is from the previous emergency prep program (Map Your Neighborhood), this video is a great watch.  Watch both the ‘Facilitator’s’ section and the ‘Neighborhood Meeting’ section.  The DVD is produced in a play-pause format and has closed-caption for the hearing impaired.

Emergency Medical Use Form
In the event of calling 9-1-1 for emergency medical assistance, this form can help provide emergency medical responders with vital information to assist them in their evaluation, both on arrival to your home and enroute to the hospital.
