  This form is required for all Architectural Applications, please submit one Buff Form per project. Contact the Architectural Office for assistance with completing this form.
• Architectural Application: Sub-HOAs (Statement by Board of Governors to Approve Application by Homeowner)
  If you live in a Sub-Association Maintained Area this form must be attached to your Architectural Application and must be signed by two Board of Governors.
• Architectural Complaint Form
  This form is for any Architectural related complaints and must be signed by the complainant. Complainant identities will be held in confidence.
• Contractor Recommendation / Review Form
  Please submit details regarding your experience with contractors hired to work at your property. This information will be kept confidential and will be used to update the OVA’s Contractor List on a regular basis.
• Firewise Assessment Request
  This form is used to request Firewise Assessments of your property. Submissions are sent to firewise@oakmontvillage.com. • Garage Sale and Estate Sale General Waiver and Indemnity
  This form is required to be completed and submitted for any garage sale or estate sale. Please see the Architectural Office for information on the maximum number of sales per year.
• Homeowner’s Association Board of Governors Application for Approval
  This form is required for Sub-Association Board of Governors who are seeking to complete project work in a common area.
• Nuisance Pet Complaint Form
  This form is for residents seeking to file a formal complaint with OVA regarding pet behavior.
• Paint Specifications Chart
  This chart is required to be completed and attached to all applications requesting to paint.
• Updates for Service Providers
  This form is directed towards service providers working in Oakmont Village. Subscribers will receive updates from OVA on our architectural and landscaping policy changes and recommendations.
